Foster Care Case Manager

What This Role Involves

As a Foster Care Case Manager, you'll deliver direct support and advocacy to children in foster care and their families. This full-time position requires you to work thoughtfully with vulnerable populations, combining professional expertise with genuine compassion to advance safe placements, family stability, and lasting positive outcomes.

Core Duties

• Assess each child and family's needs and develop comprehensive, individualized case plans aligned with their goals
• Conduct regular home visits and maintain consistent contact with children, caregivers, and household members
• Coordinate essential services—education, healthcare, mental health, and community resources—across multiple providers
• Maintain complete, accurate case documentation and comply with state and federal reporting requirements
• Partner with schools, therapists, attorneys, social workers, and community agencies to support holistic child and family wellbeing
• Monitor and advance permanency plans, whether through family reunification, kinship placement, or adoption

Who We're Looking For

• Bachelor's degree in social work, psychology, human services, or related discipline (or equivalent relevant experience)
• Valid driver's license and dependable transportation to complete field visits
• Strong written and verbal communication skills with ability to manage multiple complex cases
• Proven dedication to child welfare and family-centered, trauma-informed practice
• Ability to work both independently and collaboratively within a team environment
• Respect for diverse cultures and commitment to equitable, person-centered engagement
