Family Case Manager

The State of Indiana seeks an experienced Family Case Manager to join our workforce in Spencer, Owen County. As a Family Case Manager with the state, you'll work directly with families to coordinate support services, manage cases, and ensure eligible individuals receive the resources and assistance they need to achieve stability and improved outcomes.

What You'll Do

• Assess family circumstances, needs, and available resources to develop comprehensive case plans
• Coordinate services and support from multiple agencies and community providers
• Monitor case progress and adjust plans based on family needs and changing circumstances
• Maintain detailed, accurate case documentation and communicate with supervisors and partners
• Connect families with local resources, benefits programs, and emergency services
• Conduct home visits and attend family meetings to support engagement and accountability
• Advocate for families while maintaining professional boundaries and agency protocols

What We're Looking For

• High school diploma or GED; bachelor's degree in social work, psychology, counseling, or related field preferred
• Knowledge of Indiana's social service system and benefits programs
• Strong interpersonal and communication skills to work effectively with diverse families
• Ability to manage multiple cases, prioritize responsibilities, and meet documentation deadlines
• Commitment to ethical practice, confidentiality, and professional conduct
• Valid driver's license and reliable transportation for case visits within Owen County and surrounding areas
• Willingness to learn and grow within a state agency environment
