PATH (People Assisting the Homeless) is seeking a dedicated Case Manager for the Interim Housing team at the Highland Gardens location in Los Angeles County. This full-time position offers the opportunity to directly support individuals working toward housing stability and personal recovery.
About the Role
In this position, you'll work directly with clients experiencing homelessness across all stages—from street outreach through interim housing and into permanent supportive housing. You'll provide comprehensive, individualized support designed to help each person build the foundation for long-term stability.
Your key responsibilities will include:
- Conducting intake assessments and collaborating with clients to develop Individualized Service Plans (ISP) that address housing goals, income growth, healthcare needs, and other personal priorities
- Connecting clients with essential resources including healthcare, social services, financial assistance, vocational training, and community support systems through the coordinated entry system
- Completing standardized assessments to understand client needs and inform service planning
- Conducting 90-day progress reviews and adjusting support plans based on client outcomes and changing circumstances
- Maintaining complete, accurate, and confidential client records in accordance with HIPAA requirements and organizational policies
- Building therapeutic, trust-based relationships with clients and community partners that strengthen engagement and persistence toward housing goals
- Providing immediate intervention and safety planning during crises, helping clients develop problem-solving skills and emotional resilience
- Using motivational interviewing and trauma-informed practices in all client interactions
- Staying current with local community resources and coordinating wrap-around services that address the full spectrum of client needs

Preferred qualification: Two years of experience in case management, healthcare, homeless services, or working with vulnerable populations.

Required for all candidates: Valid driver's license and reliable transportation (driving is essential to this role); proof of insurance for personal vehicles used for work; CPR and First Aid certification (or willingness to obtain); successful completion of tuberculosis screening, background check, and drug screening; and current employment authorization in the United States.
