Master Social Worker – MSW – Part Time

You'll serve as a vital bridge between patients and our healthcare team, addressing the emotional, social, and practical factors that impact recovery and life quality.

What You'll Do

• Conduct comprehensive psychosocial assessments for dialysis patients to identify barriers to treatment adherence and optimal functioning
• Develop and implement individualized care plans addressing patients' emotional, social, and practical needs
• Provide individual and family counseling to support adaptation to chronic illness and dialysis therapy
• Participate in interdisciplinary team meetings to ensure coordinated, patient-centered care delivery
• Connect patients and families with community resources, support groups, and financial assistance programs
• Monitor patient progress and adjust interventions based on therapeutic goals and outcomes
• Maintain comprehensive documentation in compliance with regulatory and organizational standards

What We're Looking For

• Master of Social Work (MSW) degree from an accredited program
• Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential
• Prior experience providing psychosocial intervention in a healthcare or medical setting
• Strong capabilities in patient assessment and therapeutic counseling
• Ability to communicate effectively with diverse patient populations in verbal and written formats
• Demonstrated commitment to patient advocacy and person-centered care philosophy
• Capacity to work effectively within an interdisciplinary healthcare team environment
