In this hands-on clinical role, you'll deliver skilled physical therapy interventions directly in patients' homes, working Monday through Friday as part of a collaborative home health team.

What This Role Involves

As a Home Health Physical Therapist Assistant with Aveanna Healthcare, you'll work in a patient-centered, in-home environment where clinical expertise meets direct patient care. You'll execute treatment plans established by the supervising physician and physical therapist, adapting interventions to each patient's home setting. Your work impacts recovery, mobility, and independence—all while building relationships with patients and their families.

What You'll Do

• Deliver therapeutic exercises, mobility training, and functional interventions within the home health plan of care
• Provide skilled patient education on exercises, safety, and adaptive strategies for daily living
• Document patient progress, treatment responses, and observations in clinical records
• Collaborate with physicians, physical therapists, nurses, and other team members to optimize patient outcomes
• Communicate updates on patient status and treatment effectiveness to the interdisciplinary care team
• Maintain accurate schedules across your La Crosse service territory
