About the Role
As a Clinical Health Social Worker in our Emergency Department, you'll serve as a bridge between acute medical care and the broader support systems patients need. You'll meet individuals and families in acute distress, assess their psychosocial needs, and help connect them with resources, community services, and appropriate next-level care. Your clinical judgment and trauma-informed approach will be central to helping patients navigate emergencies safely and move forward in their recovery.
What You'll Do
- Assess psychosocial factors, mental health status, and barriers to care for emergency patients
- Implement crisis intervention and support strategies to stabilize patients in distress
- Coordinate discharge planning and arrange community referrals and resources
- Collaborate with physicians, nurses, and interdisciplinary teams to ensure holistic care
- Provide guidance and emotional support to patients and families during high-stress situations
- Assist with de-escalation and behavioral health crises when needed
- Maintain comprehensive clinical documentation and ensure continuity of care
- Advocate for patients' psychosocial needs and rights within the clinical environment
What We're Looking For
- Licensed Master Social Worker (LMSW) licensure
- Clinical experience in emergency medicine, behavioral health, or crisis intervention
