What You'll Do
- Conduct comprehensive evaluations and develop individualized treatment plans for SNF patients
- Perform hands-on physical therapy interventions including therapeutic exercise, manual therapy, and functional training
- Document patient progress, track outcomes, and adjust treatments based on patient response
- Collaborate with interdisciplinary care teams including nurses, occupational therapists, and physicians
- Educate patients and families on home exercise programs and discharge planning
- Maintain compliance with facility protocols, HIPAA standards, and state licensure requirements
What We're Looking For
- Current Physical Therapist license in Alabama or willingness to obtain reciprocal licensure
- Bachelor's degree minimum from an accredited PT program; DPT preferred
- Proven experience in skilled nursing or rehabilitation settings
- Strong clinical assessment and treatment planning skills
- Excellent documentation and communication abilities
- Adaptability and self-direction in fast-paced healthcare environments
