Social Work Designee

Blue Point Nursing and Rehab Center in Arlington, Baltimore is hiring a full-time Social Work Designee to provide essential psychosocial support and care coordination for our residents and their families.

About This Position

We're looking for a compassionate professional to join our interdisciplinary care team as a Social Work Designee. You'll be the bridge between residents, their families, and our clinical staff—addressing emotional, social, and practical needs while fostering a supportive environment where residents can thrive. This role is fundamental to our commitment to delivering holistic, person-centered care in our nursing and rehabilitation setting.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental concerns
• Develop and implement individualized care plans that address psychosocial needs alongside medical treatment
• Facilitate regular communication between residents, families, and the clinical team to coordinate care and resolve concerns
• Provide emotional support, counseling, and resource referrals to residents and families navigating transitions and challenges
• Connect residents with community resources, social services, and discharge planning support
• Document assessments, interventions, and progress in resident records in compliance with facility standards
• Participate in interdisciplinary team meetings to support integrated, coordinated care planning

What You Bring

• Bachelor's degree in social work or related field, or equivalent professional experience
• Demonstrated ability to assess psychosocial needs and respond with empathy and cultural sensitivity
• Strong communication and interpersonal skills to work effectively with diverse populations
• Experience in healthcare, long-term care, or social services preferred
• Knowledge of community resources, social services, and discharge planning processes
• Excellent organizational skills and attention to detail in documentation
• Ability to work collaboratively within a multidisciplinary healthcare team
