About the Role
Our Transitional Care Unit specializes in short-term inpatient recovery for adults following acute medical events and surgical procedures. You'll join an interdisciplinary team focused on restoring each patient to their highest level of independence and functionality before returning home. Working the night shift, you'll provide critical care and support during hours when attentive patient monitoring is essential to recovery outcomes.
What You'll Do
- Provide direct hands-on care to adult patients across cardiac, trauma, orthopedic, and neurological specialties
- Monitor and record vital signs, conduct patient assessments, and communicate significant changes to the clinical team
- Administer medications and treatments within your scope of practice as a Licensed Practical Nurse or Graduate Practical Nurse
- Support patients with activities of daily living, personal hygiene, and comfort measures
- Educate patients and families about recovery, discharge planning, and self-care strategies
- Maintain comprehensive and accurate patient documentation and medical records
- Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate integrated care
What We're Looking For
- Active Licensed Practical Nurse (LPN) or Graduate Practical Nurse (GPN) state license
- Current CPR/BLS certification or commitment to obtain certification
- Strong interpersonal and communication abilities
- Dedication to patient-centered care and positive patient outcomes
- Comfort working night shift in a dynamic hospital environment
- Prior nursing or healthcare experience valued; recent graduates welcome to apply
