PATH (People Assisting the Homeless) is hiring a Rapid Rehousing Case Manager at our Costa Mesa office in Orange County. This full-time position offers you the opportunity to directly support individuals navigating homelessness toward sustainable housing and self-sufficiency.
About the Role
As a Rapid Rehousing Case Manager on PATH's Interim Housing Services team, you'll work with clients across all phases of their homelessness experience—from street outreach through permanent housing. You'll combine person-centered advocacy with practical problem-solving, helping individuals build the stability and resources they need to maintain housing long-term.
What You'll Do
- Conduct comprehensive intake assessments and develop Individualized Service Plans (ISP) in partnership with clients, addressing housing goals, income stability, medical and mental health needs, substance use support, financial planning, career readiness, and social support networks
- Connect clients with community resources, public benefits, and support services that align with their ISP objectives
- Administer program-specific assessments such as the VI-SPDAT to identify housing barriers and client support needs
- Complete 90-day progress reviews to monitor goal achievement and adjust service strategies as circumstances evolve
- Maintain organized, accurate client files and program documentation in compliance with HIPAA standards and organizational policies
- Uphold strict confidentiality and professional ethics in all interactions and record-keeping
What We're Looking For
- Experience: Preferred background of two years in healthcare, case management, homeless services, or work with vulnerable populations. Candidates of all educational levels are encouraged to apply.
- Core Competencies: Analytical mindset with attention to detail; clear, effective written and verbal communication; collaborative approach to teamwork with clients and colleagues; proactive problem-solving; strong ethical judgment and professional boundaries
- Essential Requirements: Valid U.S. driver's license, reliable personal transportation, and proof of insurance; authorization to work in the United States; willingness to obtain CPR and First Aid certification if needed; pass background screening, tuberculosis test, and drug screening
