  • Develop and implement individualized plans of care focused on activities of daily living (ADLs), instrumental activities of daily living (IADLs), home safety, and fall prevention.

  • Assess for, recommend, and train patients and caregivers on adaptive equipment, assistive devices, and home modifications to support functional independence.

  • Document evaluations, progress, and clinical decisions promptly and thoroughly in Homecare Homebase (HCHB).

What You Bring

  • Strong experience or proven comfort working independently in home health settings with sound clinical judgment.

  • Ability to recognize and appropriately escalate changes in patient condition to physicians and nurses with clear documentation of your clinical reasoning.

  • Excellent verbal and written communication skills; ability to document in a way that's thorough, accurate, and defensible.

  • Reliable execution of treatment plans, visit frequencies, and patient goals with demonstrated follow-through.

  • Valid driver's license, current auto insurance, reliable transportation, and comfort traveling daily across your assigned service area to visit patients at home.

  • Physical ability to safely carry and transport basic occupational therapy equipment.

  • Solid foundation in core home health occupational therapy practice: ADL/IADL training, home safety assessment, adaptive equipment selection, and family/caregiver education.

Build the skills for this role

Prioritize active OT licensure for Michigan, strong evaluation and documentation habits, and comfort with PRN onboarding across sites. Core skills: ADL/IADL assessment, therapeutic activity design, caregiver education, and interdisciplinary communication. No certification cost data was supplied in the source list—confirm any preferred credentials (e.g., specialty CE) directly with the employer.

Prepare concise examples of PRN adaptability: covering an unfamiliar unit, prioritizing a full caseload, and documenting under time pressure. Be ready to discuss discharge planning, safety with transfers/ADLs, and how you communicate with PT/nursing. Ask Fira-health about settings served, EMR, orientation for PRN staff, and typical shift notice.

Strong fit if you are a licensed OT who wants flexible PRN hours in Oakland County and can ramp quickly across sites. Ideal if you value autonomy and variable census over a fixed full-time roster. Thin public company detail means success depends on clarifying caseload, supervision, and pay structure in the interview—not on external review data.
