Medical Social Worker (MSW)

About the Role

As a Medical Social Worker, you'll be a vital member of our primary care team, providing psychosocial assessment, care coordination, and counseling services to our patient population. You'll address the social determinants of health and connect patients with resources that support their overall wellbeing and treatment adherence.

What You'll Do

• Conduct comprehensive psychosocial assessments to identify patient needs, barriers to care, and opportunities for support
• Develop individualized care plans addressing social, emotional, and environmental factors affecting health
• Coordinate with physicians, nurse practitioners, and clinical staff to integrate social services into patient treatment plans
• Facilitate discharge planning and transitions of care, ensuring patients have resources and support upon leaving care
• Connect patients with community resources, social services, financial assistance programs, and mental health support
• Provide crisis intervention and counseling to patients and families navigating complex health challenges
• Maintain thorough, timely documentation in compliance with regulatory and clinical standards
• Collaborate with case managers and community partners to optimize patient outcomes

What We're Looking For

• Master's degree in Social Work (MSW) from an accredited program
• Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in South Carolina
• Experience in healthcare settings, particularly primary care or community health
• Strong assessment and counseling skills with diverse patient populations
• Excellent communication and interpersonal abilities
• Ability to work collaboratively within a healthcare team environment
• Commitment to addressing health equity and social determinants
