Oak Street Health New Graduate Nurse Practitioner Fellowship (Spanish Required)

Join Oak Street Health as a New Graduate Nurse Practitioner in our Casas Adobes, Pima County office. This full-time fellowship program is designed to support newly licensed nurse practitioners as they transition into independent clinical practice while serving diverse patient populations in primary care.

About the Role

This fellowship positions you to develop clinical expertise under experienced supervision while delivering comprehensive primary care to our community-based patient population. You'll work alongside interdisciplinary healthcare teams to provide evidence-based care in a collaborative, supportive environment.

What You'll Do

• Conduct patient evaluations, health history assessments, and physical examinations
• Develop and manage evidence-based treatment plans in collaboration with supervising physicians and clinical staff
• Provide preventive care, health promotion, and patient education
• Document clinical encounters and maintain accurate medical records
• Participate in quality improvement initiatives and clinical team meetings
• Build relationships with patients and provide culturally sensitive care to our Spanish-speaking communities
• Work within Oak Street Health's integrated care model to coordinate patient services

What We're Looking For

• Current, unrestricted Nurse Practitioner license
• Willingness to develop and demonstrate clinical competency under mentorship
• Fluent Spanish language skills (required)
• Strong clinical assessment and critical thinking abilities
• Commitment to patient-centered, evidence-based practice
• Ability to work effectively within a collaborative healthcare team
• Strong communication and interpersonal skills
