Home Health Physical Therapist Assistant PTA Full Time

Aveanna Healthcare seeks a dedicated Full-Time Home Health Physical Therapist Assistant to deliver in-home therapy services to patients throughout La Crosse, Wisconsin. As a PTA with our team, you'll bring patient-centered care directly to where your clients live, working closely with physicians, nurses, and other healthcare professionals to support recovery and mobility goals.

About the Role

This is a hands-on position in home-based physical therapy. You'll execute personalized treatment plans, guide patients through therapeutic exercises and activities, and help them regain function and independence in their own homes. Your work makes a tangible difference—patients see you in their living spaces, not clinical settings, creating unique opportunities to understand their real-world needs and challenges.

What You'll Do

• Perform skilled therapeutic interventions, exercises, and modalities following physician-directed plans of care
• Educate patients and families on therapy techniques, safety protocols, and home exercise programs
• Document patient progress, functional outcomes, and treatment responses accurately and promptly
• Assess patient mobility, strength, balance, and functional status during home visits
• Collaborate with interdisciplinary care teams—physicians, nurses, occupational therapists, social workers—to coordinate comprehensive care and share clinical updates
• Support patients' goals for independence and quality of life within their home environments
• Maintain professional communication with patients, families, and the medical team throughout the care process

What We're Looking For

• Current, active Physical Therapist Assistant (PTA) license in Wisconsin
• Current CPR certification
• Experience in home health, skilled nursing, or acute care settings preferred
• Strong clinical assessment and patient communication skills
• Ability to work independently and manage time across multiple home visits
• Comfort with documentation systems and electronic health records
• Reliable transportation and valid driver's license (frequent home visits throughout La Crosse)
• Professional demeanor, compassion for patients, and collaborative mindset

Schedule & Location

This full-time position runs Monday through Friday, serving the La Crosse, La Crosse County area. You'll travel between patient homes, so a reliable vehicle is essential.
