Honorhealth is hiring a Home Health Physical Therapist for full-time opportunities serving patients across Multiple locations. This role allows you to deliver skilled physical therapy care directly in patients’ homes while supporting independence and recovery.

The Opportunity

As a Home Health Physical Therapist with Honorhealth, you will provide essential rehabilitation services to individuals recovering from injury, surgery, or managing chronic conditions in the comfort of their home environment. You will work as part of a coordinated care team to help patients regain mobility, reduce pain, and improve functional outcomes across Multiple locations. This full-time Home Health Physical Therapist position combines clinical expertise with compassionate, patient-centered practice to make a real difference in community-based care.

Your Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments for home health patients to establish accurate baseline status and progress toward goals.
• Develop and implement individualized treatment plans that address mobility, strength, balance, and activities of daily living within the home setting.
• Deliver skilled physical therapy interventions, including therapeutic exercise, gait training, manual techniques, and functional training tailored to the home environment.
• Educate patients, family members, and caregivers on safe exercise programs, proper body mechanics, and strategies to prevent falls or further injury.
• Document all assessments, treatments, progress notes, and care plans accurately and in a timely manner according to regulatory and organizational standards.
• Collaborate closely with physicians, nurses, occupational therapists, and other interdisciplinary team members to ensure coordinated, goal-oriented care.
• Monitor patient response to treatment and adjust plans of care as needed to maximize functional independence and safety at home.

What You Bring

• Current state licensure as a Physical Therapist in good standing and eligibility to practice in the service area covering Multiple locations.
• Graduation from an accredited physical therapy program with the required degree for licensure.
• Solid clinical knowledge of rehabilitation techniques applicable to home health, orthopedic, neurologic, and geriatric patient populations.
• Strong interpersonal and communication skills to build rapport with patients and families and to collaborate effectively with multidisciplinary colleagues.
• Ability to work independently in the home setting, manage a caseload efficiently, and travel between patient homes as required for home health visits.
• Proficiency with electronic clinical documentation systems and familiarity with home health regulations and outcome measures.
• Valid driver’s license and reliable transportation to travel to multiple patient locations safely and on schedule.
