Your Responsibilities Perform comprehensive physical therapy evaluations and reassessments for home health patients to establish accurate baseline status and progress toward goals. Develop and implement individualized treatment plans that address mobility, strength, balance, and activities of daily living within the home setting. Deliver skilled physical therapy interventions, including therapeutic exercise, gait training, manual techniques, and functional training tailored to the home environment. Educate patients, family members, and caregivers on safe exercise programs, proper body mechanics, and strategies to prevent falls or further injury. Document all assessments, treatments, progress notes, and care plans accurately and in a timely manner according to regulatory and organizational standards. What You Bring Current state licensure as a Physical Therapist in good standing and eligibility to practice in the service area covering Multiple locations. Graduation from an accredited physical therapy program with the required degree for licensure. Solid clinical knowledge of rehabilitation techniques applicable to home health, orthopedic, neurologic, and geriatric patient populations. Strong interpersonal and communication skills to build rapport with patients and families and to collaborate effectively with multidisciplinary colleagues.
