About the Role

As a Home Health Occupational Therapist, you'll work within physician-directed plans of care to help patients regain and maintain functional abilities in their own environments. You'll be the clinical leader in home settings, collaborating closely with interdisciplinary team members including nurses, physical therapists, and social workers to ensure comprehensive, coordinated care that supports each patient's recovery and independence goals.

What You'll Do

• Conduct thorough functional assessments in patients' homes to establish baselines and identify occupational therapy needs
• Develop and implement individualized treatment plans aligned with physician orders and patient goals
• Teach patients and caregivers adaptive techniques and strategies for activities of daily living (ADL)
• Recommend and educate on adaptive equipment, mobility aids, and home modifications that promote independence and safety
• Document clinical findings, progress, and outcomes in compliance with home health documentation standards
• Communicate regularly with the interdisciplinary care team, providing clinical updates and receiving feedback to optimize patient well-being
• Evaluate home safety and recommend environmental modifications as needed

What We're Looking For

• Current, active Occupational Therapist license in Alabama
• Bachelor's degree or higher in Occupational Therapy from an accredited program
• Valid driver's license and reliable transportation for home visits
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities with patients, families, and care teams
• Ability to work independently in diverse home environments while maintaining clinical excellence
• Genuine commitment to patient-centered care and functional independence outcomes
