About the Role
As part of PATH's Interim Housing Services team, you'll work with clients across all phases of their housing journey—from street outreach to permanent placement. Your role is to support each person in breaking the cycle of homelessness by developing individualized service plans, connecting them with essential resources, and helping them build the skills and stability needed for independent living.
What You'll Do
- Conduct comprehensive intake assessments and collaboratively develop individualized service plans addressing housing goals, income, health, and personal objectives
- Connect clients with healthcare, social services, employment resources, public benefits, and housing opportunities tailored to their needs
- Administer program assessments such as the VI-SPDAT to identify strengths and barriers
- Apply motivational interviewing and trauma-informed care approaches in all client interactions
- Deliver crisis intervention and risk assessment services that build client problem-solving and coping capacity
- Maintain thorough, timely case documentation and ensure full HIPAA and PATH policy compliance
- Conduct 90-day reviews to track progress, evaluate goal achievement, and refine service plans
- Foster collaborative relationships with colleagues, clients, and community partners to strengthen engagement and program outcomes
- Stay informed about available community resources to deliver coordinated, wrap-around services
What We're Looking For
- Strong analytical skills and meticulous attention to detail in all documentation and assessments
- Clear communication abilities—both verbal and written—with diverse audiences
- Collaborative approach to teamwork and stakeholder engagement
- Proactive problem-solving with creativity and willingness to take initiative
- Commitment to ethical practice and maintaining professional boundaries
- Preferred: Two years of experience in case management, healthcare, homeless services, or direct support roles with vulnerable populations
- Valid California driver's license and reliable transportation (driving is essential to this role)
- Current or willingness to obtain CPR and First Aid certification
- Ability to pass background screening, tuberculosis test, and drug screening
- U.S. work authorization and ability to work within California
