What You'll Do
- Conduct thorough intake assessments and develop individualized service plans addressing housing, income stability, medical care, mental health, substance use concerns, financial resources, employment, and social support networks
- Provide targeted referrals, resource connections, and coordinated support to help participants achieve their service plan goals
- Administer validated assessment tools, including the VI-SPDAT, to identify client strengths and service needs
- Monitor progress, complete 90-day reviews, and refine service strategies to support clients' advancement toward housing stability

Mandatory Requirements
- Current US employment eligibility authorization
- CPR and First Aid certification (PATH can support your training if needed)
- Clearance of tuberculosis screening, background check, and drug screening (upon conditional job offer)
- Valid driver's license and reliable personal vehicle with current insurance documentation
- Ability to meet PATH's vehicle insurance coverage requirements
Work Schedule Options
Select from flexible scheduling to support work-life balance:
- Sunday–Thursday, 7:00 am–3:30 pm
- Sunday–Thursday, 3:00 pm–11:30 pm
- Tuesday–Saturday, 7:00 am–3:30 pm
- Tuesday–Saturday, 3:00 pm–11:30 pm
Compensation and Benefits
This full-time, non-exempt position offers an hourly wage of $23.00–$28.00 based on your experience and qualifications. Benefits include comprehensive medical, dental, and vision coverage; paid vacation and sick time; paid holidays; and a retirement plan.
