About the Role

This full-time position serves clients across the entire housing continuum—from street outreach through permanent supportive housing. You'll use a personalized, client-centered approach to help each person develop a concrete plan for stable housing and self-sufficiency. The role emphasizes building trust, removing obstacles to care, and coordinating a network of health, social, and employment services that support independent living.

What You'll Do

  • Conduct comprehensive intake assessments and work collaboratively with clients to create Individualized Service Plans addressing housing goals, income, healthcare, mental health, substance use, and personal priorities
  • Connect clients to healthcare, social services, employment support, and community resources; follow up regularly to ensure access and continued effectiveness
  • Use evidence-based assessment tools to evaluate client strengths, barriers, and progress toward identified goals
  • Facilitate 90-day progress reviews and adjust service plans based on outcomes and evolving needs
  • Maintain accurate, timely case documentation in compliance with program standards, HIPAA requirements, and agency policies
  • Provide crisis intervention, risk assessment, and supportive counseling during periods of acute instability or setback
  • Apply trauma-informed practices and motivational interviewing techniques consistently in client interactions
  • Build collaborative relationships with colleagues, partner agencies, and community systems to coordinate integrated care and address structural barriers
  • Participate actively in staff meetings, required training, and team initiatives; demonstrate commitment to diversity, equity, and inclusion in service delivery and workplace culture

What We're Looking For

  • Strong writing and verbal communication skills; ability to distill complex situations into clear documentation
  • Analytical mindset with meticulous attention to detail and accuracy
  • Genuine empathy and commitment to working with individuals experiencing homelessness and other vulnerable populations
  • Creative problem-solver who demonstrates initiative and tailors approaches to individual client circumstances
  • Collaborative team player who values diverse perspectives and works effectively across disciplines
  • Reliable professional with strong personal integrity and ability to maintain appropriate boundaries
  • Two or more years of experience in case management, healthcare, homeless services, or supporting vulnerable populations (preferred but not required)

Mandatory Requirements

  • Valid driver's license, reliable transportation, proof of insurance, and ability to qualify for PATH's vehicle coverage
  • Willingness to obtain CPR/First Aid certification before start date
  • Pass a background screening, tuberculosis test, and drug screening as conditions of employment
  • U.S. work authorization (we cannot sponsor or assume sponsorship of employment visas)
  • You are not currently receiving PATH services and have not received services within the past two years
