Position Summary
We are building a strong care coordination team dedicated to serving individuals in our community. As our Care Coordination Supervisor, you will oversee and develop the care coordination function within Region I, ensuring that the people we serve receive coordinated, compassionate support tailored to their unique needs. This supervisory role sits at the heart of our community-focused approach, where you'll lead a team connecting individuals with behavioral health services and community resources right in their homes and neighborhoods.
Key Responsibilities
- Supervise and mentor a team of care coordination professionals, fostering excellence in service delivery
- Ensure care coordination activities align with clinical standards and organizational best practices
- Oversee direct service delivery to individuals receiving behavioral health support within Region I
- Monitor team performance, caseload management, and client outcomes
- Collaborate with clinical and community partners to connect clients with essential resources and services
- Support staff in addressing barriers and advocating for clients' behavioral health goals
- Maintain comprehensive documentation and reporting on care coordination activities
- Contribute to program development and continuous improvement initiatives
Who We're Looking For
- Supervisory or leadership experience in behavioral health, social services, or a related field
- Demonstrated ability to lead teams and develop staff talent
- Strong communication and interpersonal skills for working with diverse populations
- Knowledge of behavioral health services, treatment modalities, or community resource systems
- Ability to manage multiple priorities and maintain attention to detail in documentation
- Commitment to person-centered, trauma-informed care practices
- Valid driver's license and reliable transportation for regional service delivery
