What This Role Involves

The LPN RAI Coordinator position with Sasllc centers on guiding the full cycle of resident assessments so that care teams at Archway Transitional Care - Macon, GA 31206 receive timely, complete, and regulation-ready data. You serve as the key point of contact for MDS accuracy, schedule adherence, and seamless hand-offs between clinical departments, ensuring every assessment supports individualized care and meets federal requirements. Day-to-Day Responsibilities

• Plan, track, and complete RAI/MDS assessments according to established timelines and resident needs
• Gather clinical input from nurses, therapy staff, and other disciplines to produce accurate coding and documentation
• Review records for completeness, resolve discrepancies, and submit finalized assessments on schedule
• Update assessments after significant resident changes and maintain audit-ready files
• Support care-plan meetings by presenting assessment findings and recommending next steps
• Stay current with CMS RAI guidelines and apply updates consistently across daily workflows
• Coordinate with billing and quality teams when assessment data affects reimbursement or quality metrics

Who We're Looking For

• Active, unrestricted LPN license valid in Georgia
• Hands-on familiarity with RAI processes, MDS tools, and nursing assessment documentation preferred
• Solid grasp of clinical terminology and the ability to interpret interdisciplinary notes
• Strong organizational skills for juggling multiple assessment deadlines
• Clear written and verbal communication that builds trust with the care team
• Dependable computer proficiency for electronic health record entry and reporting
