About This Opportunity
Our talent team is representing a confidential healthcare organization in a search for an experienced Care Navigator with deep knowledge of VA benefits and Veteran healthcare systems. This fully remote role combines direct care coordination with program development responsibilities, allowing you to support Veterans while helping shape how care navigation is delivered across a growing team.

Responsibilities
You'll help Veterans understand and navigate available resources, including VA healthcare, benefits programs, community care options, and services addressing both healthcare and social needs. Your work will involve direct Veteran engagement as well as serving as an internal resource when complex cases require specialized knowledge.
- Guide Veterans through healthcare navigation, benefits access, behavioral health services, and community resources
- Support care coordinators across the team when Veteran cases become complex or require VA-specific expertise
- Determine appropriate referrals and escalations for medically and behaviorally complex situations
- Document cases thoroughly within care management and electronic health record systems
- Develop and refine Veteran-specific care coordination workflows and protocols
- Identify program gaps and opportunities for improvement as the initiative evolves
- Contribute to training materials and team education on VA healthcare, benefits navigation, and common barriers to care
- Collaborate with care coordination and program leadership to strengthen team capabilities
What We're Looking For
We're seeking a professional whose background brings together Veteran or VA experience, care coordination expertise, and increasing program responsibility. Your knowledge should go beyond individual case management to include systems thinking and the ability to translate your experience into resources that help others.
- 5+ years of experience as a clinical social worker, care coordinator, care navigator, or similar role, with demonstrated progressive program management experience
- Direct experience working with Veterans and/or within the VA system
- 1+ years of successful remote work experience with a dedicated home-office environment
- In-depth knowledge of VA healthcare, Veteran benefits and resources, behavioral health systems, and social determinants of health
- Experience supporting medically and/or behaviorally complex populations
- Proficiency documenting cases within care management or electronic health record systems
- Strong communication, collaboration, and problem-solving abilities
- Experience with motivational interviewing techniques
- Must reside in the Central Time Zone (Texas, Alabama, Mississippi, Louisiana, or Tennessee)
A social work license is not required, though candidates with credentials such as LMSW or similar professional backgrounds are encouraged to apply. Fluent Spanish is helpful but not required.

Position Details
This is a full-time, 40-hour-per-week remote position working Monday through Friday during normal Central Time business hours. Travel is not required. This is a permanent employment opportunity.
Pay
Our client has indicated an anticipated salary range of $80,000 to $100,000 annually, with potential flexibility based on experience and background. Specific compensation details will be discussed during the interview process.
