LTSS Service Care Coordinator

About This Opportunity

We are representing a confidential healthcare organization seeking an LTSS Service Care Coordinator to join their team. This remote position is based in Texas and involves coordinating holistic care management activities for long-term services and supports members, with a particular focus on foster care and medically fragile children and adolescents.

Location: Texas (remote with 25% travel for face-to-face assessments)

Employment Type: Full-time

Responsibilities

• Evaluate member needs and available resources to recommend and facilitate optimal care outcomes
• Assist in developing ongoing long-term care plans and service plans, identifying necessary providers, specialists, and community resources
• Coordinate between members, families/caregivers, and care provider teams to ensure accessibility of identified services
• Provide resource support for various needs including employment, housing, participant direction, independent living, and foster care based on service assessments
• Monitor care plans, member status, and outcomes, providing recommendations based on identified needs
• Conduct on-site visits to assess member needs and collaborate with providers or resources as appropriate
• Collect, document, and maintain member information and care management activities ensuring compliance with state, federal, and third-party payer regulations
• Provide education to members and families/caregivers on procedures, healthcare provider instructions, service options, referrals, and benefits
• Interact with long-term care healthcare providers and partners to ensure member needs are met
• Provide feedback to leadership on opportunities to improve quality of care and service delivery

What We're Looking For

• Bachelor's degree and 1 year of related experience, or equivalent experience through applicable knowledge and skills
• Direct experience working with individuals who have disabilities and/or vulnerable populations with chronic or complex conditions, including children and young adults, within three of the last five years
• Previous case management experience in foster care (highly preferred)
• Experience working with medically fragile children and adolescents (highly preferred)
• Must currently reside in Texas
• Ability to travel approximately 25% for face-to-face assessments
• Strong coordination and communication skills
• Knowledge of long-term care services and supports
