RN Care Manager

Overview

Our talent team is representing a confidential healthcare organization seeking a RN Care Manager to join their community-based care coordination team. This is a remote role with field visits, supporting individuals enrolled in a state-managed care program across Wisconsin. You'll work Monday through Friday with no nights or weekends, providing meaningful clinical coordination and member education while maintaining true work-life balance.

Responsibilities

• Assess members with focus on health and safety needs to develop comprehensive care plans
• Conduct regular health assessments and coordinate with acute and primary care providers
• Educate members on disease processes, nutrition, exercise, and lifestyle modifications to support informed decision-making
• Document member interactions within required state contract timeframes
• Participate in monthly member phone contacts and travel for in-person visits in members' settings (approximately 40% travel)
• Complete record reviews for reassigned members and identify areas requiring remediation
• Support the care team through documentation, task assignment, and member transitions
• Accept complex member assignments requiring advanced clinical judgment
• Ensure continuity of care by coordinating handoffs and follow-up needs

What We're Looking For

Required:

• Associate degree in nursing
• Active Wisconsin Registered Nurse license in good standing
• Valid state driver's license and personal vehicle liability insurance (minimum 100/300/100 limits)
• Ability to work from a dedicated home office space with reliable internet (minimum 25 Mbps download, 10 Mbps upload)
• Willingness to complete TB screening as part of patient-facing role requirements

Preferred:

• 3+ years of care management experience
• Residence within 45 minutes of the Milwaukee, WI coverage area
• Previous experience with state-managed care programs or long-term care coordination

Schedule: Monday–Friday, 8:00 AM – 4:30 PM CST. This is a full-time (40 hours/week) position with approximately 40% travel for member visits. Mileage reimbursement is provided for all work-related travel.
