Behavioral Health Intensive Care Management Coordinator (Low-Country, SC)

Our talent team at Get A Job.ai is representing a confidential health insurance organization in their search for a Behavioral Health Intensive Care Management Coordinator in the Low-Country region of South Carolina, preferably Beaufort County.

This full-time position (40 hours/week, Monday–Friday, 8:30am–5:00pm EST) is fully remote but requires significant travel within your assigned South Carolina region to conduct member-facing patient visits. Mileage reimbursement is provided after mile 50 at 70 cents per mile, with a travel limit of 3 hours one way.

Responsibilities

• Serve as an Intensive Case Manager (ICM) for members identified with specific behavioral health conditions, including Serious Mental Illness (SMI), Trauma and PTSD, Substance Use Disorders (SUD), Child and Adolescent Behavioral Health, and Cognitive Disorders
• Perform primary functions of assessment, planning, facilitation, coordination, monitoring, evaluation, and advocacy of assigned members through required in-person contact
• Complete in-person assessments in the member's residence or least restrictive environment
• Collaborate and communicate with the member, member's family/caregiver, and other healthcare professionals involved in the member's care
• Ensure care coordination and case management activities are aligned with members' risk levels and identified strengths, preferences, needs, and desired outcomes
• Participate in meetings concerning the member's care and utilize all data systems containing patient information appropriately
• Develop strategies to increase effectiveness of and participation in health management programs
• Establish and maintain effective communications with provider network, group representatives, and members
• Conduct research and analysis of reports (monthly, quarterly, annually) and adhere to NCQA requirements, state/federal regulations, and CMSA standards
• Monitor member compliance and success rate, identify quality improvement opportunities, and develop patient/provider educational materials

What We're Looking For

Required Education:

• Bachelor's degree from an accredited university or college in Nursing, Psychology, Social Work, Early Childhood Education, Child Development, or a related field with a minimum of 45 documented training hours related to specialty area issues and treatment
• Master's degree may reduce experience requirements

Required Experience:

• Four years of recent clinical experience in a defined Behavioral Health specialty area (SMI, Trauma/PTSD, SUD, Child/Adolescent Behavioral Health, or Cognitive Disorders); OR
• Four years of Behavioral Health utilization review/case management/clinical or combination; 2 of the 4 years must be clinical
• At least 3 years' experience working with people with specified health conditions (2 years if master's degree)
• Direct experience working with individuals with mental illnesses strongly preferred

Required Licensure (one of the following):

• Active, unrestricted RN or LPN licensure in South Carolina or active compact multistate license (NLC)
• Active, unrestricted LBSW, LMSW, or LISW in South Carolina or with state compact
• Active, unrestricted LPC, Licensed Marriage and Family Therapist, LAC, LPES, or Licensed Psychologist in South Carolina or with state compact

Required Skills:

• Working knowledge of Microsoft Office (word processing, spreadsheet, database, presentation software)
• Understanding of insurance and benefit programs as it relates to health management coverage
• Knowledge of the quality improvement process and national case management standards
• Familiarity with evidence-based assessments
• Ability to work independently, prioritize effectively, and make sound decisions
• Demonstrated customer service, organizational, and presentation skills
• Ability to handle confidential or sensitive information with discretion
• Valid driver's license and ability to drive; availability for occasional nights and weekends
