Fira-health is seeking a Physical Therapist Assistant (PTA) to deliver skilled home-based physical therapy in Collin County, Texas. This full-time position offers you the opportunity to make a direct impact on patient outcomes and independence.
The Opportunity
As a PTA with Fira-health, you'll be central to our mission of expanding access to quality home health care. Working in patients' homes, you'll provide direct patient care under the supervision of a licensed Physical Therapist, executing treatment plans focused on restoring function, enhancing safety, and enabling independence. You'll work Monday–Friday with occasional weekend visits as needed, traveling across your assigned service area in Collin County.
Your Responsibilities
- Deliver skilled physical therapy services in patients' homes, following the supervising PT's evidence-based plan of care
- Focus treatment on functional mobility outcomes: gait training, balance and fall prevention, safe transfers, and strengthening exercises
- Track measurable progress and communicate clinical observations, concerns, and changes in patient condition to the PT and care team
- Educate patients and their caregivers on home exercise programs and practical strategies to maintain gains between visits
- Conduct and document home safety assessments, providing recommendations to reduce fall risk and improve accessibility
- Coordinate with the broader care team—including nurses, physicians, and other providers—to ensure aligned, comprehensive care
- Document all visits promptly in Homecare Homebase, the agency's electronic health record system
Who We're Looking For
- Valid, active Physical Therapist Assistant license in Texas (in good standing)
- Current driver's license and willingness to drive daily within your assigned territory
- Demonstrated clinical judgment and comfort working independently in the field within PTA scope and under PT oversight
- Strong written and verbal communication skills; ability to document clearly and escalate concerns appropriately
- Competency in core home health PTA skills: safe mobility assistance, gait and balance training, therapeutic exercise, and home safety education
- Ability to carry and transport basic therapy equipment as part of your daily work
- Reliable, organized approach to managing patient schedules, treatment plans, and documentation
- Nice-to-have: Previous experience in home health settings, familiarity with Homecare Homebase or similar home health records systems, or specialized background in orthopedic or neurological populations
