RN, Minimum Data Set (MDS) – FT

Good Samaritan is seeking a skilled Registered Nurse to join our long-term care team in Bancroft, Kingsbury County, specializing in Minimum Data Set (MDS) assessment and documentation for our senior resident population.

Position Summary

As an RN focused on Minimum Data Set functions, you'll play a critical role in our commitment to accurate, comprehensive resident assessments. Good Samaritan, a faith-based not-for-profit organization, has served seniors with dignified housing and quality care for over 100 years. This full-time position offers the opportunity to advance resident outcomes through meticulous MDS documentation, care planning, and quality assurance in our Bancroft community.

Core Duties

• Complete and maintain accurate Minimum Data Set assessments within required timeframes, ensuring compliance with regulatory standards
• Conduct comprehensive resident health evaluations and interviews to gather assessment data
• Document clinical findings, functional status, and care needs in the resident medical record and MDS system
• Collaborate with the interdisciplinary care team to develop individualized care plans based on MDS findings
• Review and update MDS data to reflect changes in resident condition and care requirements
• Maintain knowledge of current MDS guidelines and regulatory requirements; participate in ongoing compliance training
• Communicate assessment findings clearly to physicians, care staff, and families

What You Bring

• Active RN license in good standing
• Demonstrated experience with MDS assessment and documentation in long-term care or skilled nursing settings
• Strong clinical assessment and communication skills
• Ability to work independently and meet strict regulatory timelines
• Proficiency with electronic health records and data entry systems
