What This Role Involves
As a Community Engagement Specialist, you're the bridge between Zócalo Health members and accessible, respectful healthcare. Your focus is on identifying eligible individuals, initiating meaningful contact, and removing barriers to care. You'll work flexibly to reach people on their terms—at home, at community events, and in partnership spaces—ensuring no one has to navigate healthcare complexity alone.
Core Duties
- Conduct in-person and phone outreach to prospective members using health plan eligibility data and build genuine engagement.
- Schedule intake appointments and support members through enrollment documentation and required paperwork.
- Record all outreach interactions and enrollment progress accurately in Athena EHR and internal tracking systems.
- Represent Zócalo Health professionally at community coalition meetings, health events, and collaborative gatherings in Amador County.
- Build and maintain relationships with community-based organizations, coalitions, and local partners to strengthen our reach and community trust.
- Follow up with event attendees and referrals via phone calls and materials to move individuals toward enrollment.
What You Bring
- Full fluency in English and Spanish, both spoken and written, with ability to communicate across cultures with sensitivity.
- One to three years of work experience in healthcare settings, community health, community organizing, or similar field-based roles serving diverse populations.
- Proven ability to engage respectfully with people from varied backgrounds and provide culturally responsive support.
- Strong organizational skills, attention to detail, and comfort navigating electronic health records systems (Athena familiarity preferred).
- Reliable personal transportation and flexibility to work evenings and weekends as needed to attend events and meet community members.
- Authorization to work legally in the United States.
