About the Role

As a Case Manager in our Interim Housing program, you'll work directly with clients at all stages of homelessness—from street outreach through permanent housing—helping them build stable lives through personalized support and resource coordination. Your work centers on individualized service delivery, where you'll partner with each person to develop goals addressing housing stability, income growth, health, and overall wellbeing.

What You'll Do

  • Conduct intake assessments and create Individualized Service Plans (ISPs) tailored to each client's housing, income, health, mental health, and vocational goals
  • Connect clients with healthcare, social services, and community resources that advance their plan goals
  • Complete program-required assessments (including VI-SPDAT) to understand client needs and barriers
  • Perform 90-day plan reviews and ongoing evaluations to track progress and adjust strategies
  • Maintain accurate, confidential case documentation and participant files in compliance with HIPAA and PATH policy
  • Provide crisis intervention and risk assessment, building clients' capacity for independent problem-solving and coping
  • Apply motivational interviewing and trauma-informed approaches in all client interactions
  • Stay current on Los Angeles County community resources to deliver comprehensive, coordinated support

What We're Looking For

We seek someone with strong collaboration skills who can build trust with clients, colleagues, and community partners. You think critically, communicate clearly both verbally and in writing, and take initiative to solve problems creatively. You pay close attention to detail, maintain high ethical standards, and approach work with integrity. Experience in case management, healthcare, homeless services, or work with vulnerable populations is preferred (two years or more), but all experience levels are welcome to apply.

Requirements & Logistics

  • Valid work authorization in the US
  • Current CPR/First Aid certification (or willingness to obtain)
  • Pass background check, tuberculosis test, and drug screening
  • Valid driver's license and reliable transportation; proof of insurance and ownership required; must qualify for PATH's vehicle insurance (driving is essential to the role)
  • Monday–Friday, 8:30 a.m.–5:00 p.m. in-office schedule
