We're seeking a Nurse Practitioner (APRN) or Physician Assistant for our specialized CHF Clinic in Hebron, Tolland County, Connecticut, in a full-time capacity where your clinical expertise directly improves patient outcomes.

You'll deliver evidence-based cardiovascular care within Connecticut's most comprehensive healthcare network, serving patients who depend on expert heart failure management. What You'll Do

• Perform comprehensive patient assessments and diagnostic evaluations for congestive heart failure and related cardiovascular conditions
• Develop individualized treatment plans aligned with current heart failure management guidelines
• Prescribe, monitor, and adjust pharmacological therapies to optimize therapeutic outcomes
• Order, interpret, and integrate diagnostic testing, imaging studies, and laboratory results into clinical decision-making
• Educate patients and families on disease management, medication adherence, lifestyle modification, and self-monitoring strategies
• Conduct ongoing patient follow-up visits and modify interventions based on clinical response and disease progression
• Document clinical encounters and maintain accurate, timely electronic health records
• Coordinate multidisciplinary care with cardiologists and ancillary healthcare professionals

What We're Looking For

• Master's degree with current, unrestricted state licensure as a Nurse Practitioner (APRN) or Physician Assistant
• Prior experience in cardiology, cardiovascular nursing, or heart failure management strongly preferred
• Proficiency in clinical assessment, diagnostic reasoning, and patient evaluation
• Knowledge of contemporary heart failure therapeutics and evidence-based clinical practice standards
