Physical Therapist – Home Health – Spirit Lake | Part-Time

Sanford Health is seeking a compassionate, licensed Physical Therapist to join our Home Health team in the Spirit Lake area on a part-time basis, delivering personalized rehabilitation care directly in patients' homes.

As a Physical Therapist – Home Health – Spirit Lake (Part-Time) with Sanford Health, you will bring expert therapeutic care to individuals recovering in the comfort of their own residences. Key Responsibilities

• Assess patients in the home setting and design individualized treatment plans aligned with each person's recovery goals.
• Deliver hands-on physical therapy interventions to restore mobility, strength, and independence.
• Educate patients and caregivers on safe techniques, exercises, and home-based care strategies.
• Track progress, adjust therapy programs as needed, and maintain accurate clinical documentation.
• Coordinate closely with the broader home health care team to support seamless, patient-centered outcomes.

Qualifications

• Current licensure (or eligibility for licensure) as a Physical Therapist in the applicable state.
• Graduate of an accredited physical therapy program.
• Strong clinical judgment and the ability to work independently within a home health environment.
• Excellent communication skills and a genuine commitment to compassionate, patient-focused care.

Compensation

• Pay range of $37.50 – $62.00 per hour, based on experience and qualifications.
• $2,500 sign-on bonus.
• Day shift; approximately 20 scheduled hours per week. This is a non-union position.
