About the Role
We're recruiting an experienced Physical Therapist to deliver skilled rehabilitation services in a Skilled Nursing Facility setting. This travel assignment combines clinical impact with the scheduling flexibility many therapists seek while serving patients throughout the greater Stanford and Palo Alto region. You'll work 40 hours per week in an 8-hour daily shift structure, providing direct patient care and collaborating across an interdisciplinary care team.
What You'll Do
- Conduct comprehensive patient evaluations and develop individualized physical therapy treatment plans
- Deliver evidence-based therapeutic interventions to restore mobility, strength, and functional independence
- Document clinical assessments, progress notes, and outcomes in compliance with SNF standards
- Educate patients and families on prescribed exercises and home rehabilitation programs
- Collaborate with nursing, occupational therapy, and care management staff in care conferences
- Support discharge planning and community reintegration for patient success
What We're Looking For
- Current Physical Therapist license in California
- Demonstrated experience in geriatric or post-acute care environments
- Strong clinical assessment and treatment planning abilities
- Comfort working independently and as part of a multidisciplinary team
- Professional flexibility for travel-based clinical assignments
- Patient-focused approach to rehabilitation and recovery
