About the Role
In this Social Worker position with the ADRC's Cognitive Care Network, you will play a vital role in bridging clinical care and research. You'll develop professional relationships with healthcare providers—including physicians, nurse practitioners, and other clinical professionals—to advance our mission of supporting individuals with cognitive impairment and expanding knowledge about Alzheimer's disease and related conditions.
What You'll Do
- Build and maintain relationships with primary care and specialist physicians, nurse practitioners, and other healthcare providers
- Support patient recruitment and engagement in Alzheimer's disease research and care initiatives
- Connect individuals and families affected by cognitive impairment with appropriate resources and services
- Provide social work assessment and support within the research and clinical setting
- Collaborate with interdisciplinary teams to coordinate care and support services
- Facilitate communication between patients, families, providers, and research staff
- Maintain accurate documentation and records in accordance with healthcare standards
What We're Looking For
- Valid social work licensure or credentials as required by Kansas state regulations
- Experience in healthcare, gerontology, or clinical social work, or demonstrated commitment to working with older adults and cognitive health
- Strong relationship-building and communication skills across diverse settings and populations
- Ability to organize and prioritize multiple responsibilities effectively
- Compassion for individuals living with Alzheimer's disease and their families
