Role Overview

This position will be posted for a minimum of 5 days. The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. Our philosophy emphasizes the right to adequate housing and health care, aiming to remove barriers that restrict access to these rights.

What You'll Do

• Work collaboratively within the Integrated Health Care for the Homeless team.
• Provide co-management of transitional care services for patients transitioning from acute care settings to home or community-based care.
• Assess, plan, implement, and evaluate patient care plans in collaboration with interdisciplinary teams.
• Ensure compliance with all regulatory requirements and best practices in nursing care delivery.

Requirements

• Registered Nurse (RN) license in the state of Colorado.
• Minimum 2 years of experience in transitional or acute care settings preferred.
• Strong knowledge of patient assessment, diagnosis, and treatment planning.
• Excellent communication and interpersonal skills to work effectively with patients, families, and interdisciplinary teams.
