About This Position

This role is a full-time, on-site position with Coloradocoalition. The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. As a PATH Case Manager, you will be part of a team dedicated to assisting clients in achieving their goals related to housing stability.

Day-to-Day Responsibilities

  • Assist clients experiencing or at risk for homelessness in identifying and accessing appropriate services and resources
  • Develop and maintain case management plans with clients, ensuring they have the support necessary to achieve their goals
  • Provide direct support to clients, including assistance with housing applications, budgeting, and other life skills training
  • Collaborate with a multidisciplinary team of professionals to ensure comprehensive care for clients
  • Document client interactions and progress in compliance with agency policies and procedures

Skills & Qualifications

  • Bachelor's degree in social work, psychology, or related field; master’s preferred
  • Experience working with homeless populations or at-risk individuals
  • Strong case management skills and ability to build rapport with clients
  • Excellent communication and organizational skills
  • Familiarity with housing assistance programs and resources
  • Proficiency in Microsoft Office Suite and electronic health record systems
