Registered Nurse (RN) Case Manager – Hospice – FT

What This Role Involves

As an RN Case Manager in our hospice program, you'll be at the heart of our care delivery, serving as the primary clinical coordinator for your patient caseload. You'll partner directly with patients, families, and an interdisciplinary team of physicians, social workers, and support staff to craft individualized care plans that honor each person's wishes and priorities. Your clinical expertise and compassionate presence make a profound difference during one of life's most important transitions.

Key Responsibilities

• Conduct comprehensive patient assessments and develop evidence-based care plans aligned with hospice philosophy and standards
• Perform regular patient and family visits to monitor symptom management, medication effectiveness, and psychosocial needs
• Coordinate with the interdisciplinary care team to ensure seamless communication and consistent, high-quality support
• Educate patients and families about the hospice process, pain management options, and available resources
• Document patient progress, clinical changes, and interventions according to regulatory requirements and organizational standards
• Advocate for patient preferences and goals throughout their care journey
• Respond to urgent clinical needs and communicate effectively with physicians regarding treatment adjustments

What We're Looking For

• Current RN licensure
• Clinical experience in hospice, palliative care, or a related acute or long-term care setting
• Strong assessment and critical thinking skills with the ability to identify subtle changes in patient status
• Excellent communication and interpersonal abilities for working with diverse patients, families, and care teams
• Genuine compassion, patience, and emotional resilience in end-of-life care
• Ability to manage a patient caseload independently while collaborating effectively with a team
• Commitment to maintaining detailed, accurate clinical documentation
