Case Manager – Carrefour – Part-Time Weekends

Hrhs is hiring a Case Manager for our Horizons Reno County location to support clients through comprehensive case management services. This is a part-time weekend position offering flexible scheduling for those seeking meaningful work in community health.

Position Summary

As a Case Manager at Horizons Reno County, you'll provide direct, compassionate support to clients and their families, coordinating care and resources to help achieve positive outcomes. This role requires weekend availability and works closely with interdisciplinary teams to connect individuals with services and support systems in our community.

What You'll Do

• Conduct comprehensive client assessments to identify needs, strengths, and service goals
• Develop and implement individualized care plans aligned with client objectives
• Coordinate access to social services, healthcare, and community resources
• Maintain detailed, accurate documentation of client interactions and progress
• Serve as a liaison between clients, families, and service providers
• Monitor client progress and adjust services as needed
• Support clients in problem-solving and accessing emergency or crisis resources
• Participate in team meetings and contribute to treatment planning

Skills & Qualifications

• High school diploma or equivalent required; some college or prior case management experience preferred
• Excellent communication and interpersonal skills with diverse populations
