Key Responsibilities

• Conduct thorough initial and ongoing nursing assessments in patients’ homes to identify medical, functional, and psychosocial needs.
• Design, revise, and implement individualized plans of care in collaboration with physicians, therapists, and other home health partners.
• Coordinate and supervise visits by licensed practical nurses, home health aides, and therapy staff to ensure seamless service delivery.

• Active, unencumbered registered nurse license valid in Texas.
