About the Role
As a Home Health Physical Therapist with Assisted Home Health & Hospice in Lompoc, you will deliver one-on-one rehabilitation care to patients recovering in the comfort of their own homes. This position offers flexible Per-Diem or Full-Time options and a per-visit pay structure: $146 per Start of Care (SOC), $101 per Evaluation, and $91 per Routine visit. It's a meaningful opportunity to make a genuine difference in the lives of the people you serve.
Key Responsibilities
- Conduct in-home physical therapy evaluations and start-of-care assessments for Home Health patients.
- Design and carry out individualized treatment plans that support recovery, mobility, and independence.
- Provide routine therapy visits across the Lompoc and greater Santa Barbara County service area.
- Track patient progress, adjust care plans as needed, and maintain accurate clinical documentation.
- Work alongside the broader care team to support safe, positive outcomes at home.
Qualifications
- Licensed Physical Therapist (PT) eligible to practice in California.
- Comfortable working independently in a home health setting.
- Reliable transportation to travel between patient homes in the Lompoc area.
