About the Role
As a per diem Certified Home Health Aide, you'll provide essential personal care and support services to patients in their homes. This position covers the western region of our service area, with approximately 5–10 visits weekly. Your work enables clients to maintain independence and dignity while receiving compassionate, professional care in a familiar home environment.
What You'll Do
- Assist patients with activities of daily living, including bathing, grooming, dressing, and personal hygiene
- Support mobility and ambulation according to physician orders and safety protocols
- Monitor vital signs and report changes or concerns to supervisory nursing staff
- Maintain clean, safe home environments for patients
- Provide emotional support and companionship during care interactions
- Document patient interactions and care activities accurately
- Collaborate with interdisciplinary care teams to support individualized care plans
What We're Looking For
- Current California Department of Public Health Certified Home Health Aide certification
- Proven experience in direct patient care within home or clinical settings
- Strong communication and interpersonal skills
- Ability to work independently and follow care instructions with precision
- Physical capability to assist patients safely, including lifting and transferring
- Reliable transportation and valid driver's license
- Compassion, patience, and commitment to person-centered care
