Assisted Home Health & Hospice is hiring a part-time Physical Therapist (PT) for our Home Health team serving Sun City and the greater Peoria area in Maricopa County, Arizona. About the Role

As a Home Health Physical Therapist with Assisted Home Health & Hospice, you'll deliver one-on-one rehabilitative care in patients' homes across Sun City and surrounding Maricopa County communities. This part-time role gives you the flexibility to focus on what matters most: helping people recover mobility, regain strength, and stay safely independent where they live.

Key Responsibilities

• Assess patients in the home setting and design individualized physical therapy treatment plans.
• Provide skilled therapeutic interventions to improve strength, balance, mobility, and overall function.
• Educate patients and family caregivers on home exercise programs, safety, and fall prevention.
• Track patient progress and adjust care plans to support recovery goals.
• Document evaluations and visits accurately and in line with home health regulations.
• Coordinate with physicians, nurses, and the broader care team to ensure continuity of care.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist (PT) license to practice in Arizona.
• Ability to travel to patient homes throughout the Sun City and Peoria area of Maricopa County.
• Strong clinical judgment, compassion, and communication skills.
• Prior home health experience is a plus but not required.
