Outpatient LPN, Get A Job.ai Physician Clinical Support

About This Opportunity

Our talent team is representing a confidential healthcare organization in their search for an Outpatient LPN to join a centralized patient support operation. This is a Monday through Friday role with no weekends or holidays, scheduled from 9:00 a.m. to 5:30 p.m. You'll be part of a newly established team focused on medication refill coordination and patient care support across primary care settings.

We're looking for someone who thrives in collaborative, technology-forward environments and is excited about contributing to process improvements in a fast-paced clinical support role. This position offers the opportunity to shape how patient medication management services are delivered while working alongside a multidisciplinary healthcare team.

Responsibilities

• Perform initial and ongoing health status assessments appropriate to the patient population, collecting and documenting subjective and objective data
• Identify and communicate assessment findings to appropriate team members through referrals, huddles, reports, and care conferences
• Collaborate assessment findings with providers and healthcare team members as needed
• Document planned nursing interventions based on nursing diagnoses and collaborative problems to achieve patient-appropriate outcomes
• Coordinate care plans and individualize medical management plans to promote cost-effective, coordinated care
• Carry out and document interventions using scientific knowledge, critical thinking, and effective interpersonal skills
• Evaluate and document patient responses to nursing interventions at appropriate intervals
• Delegate tasks in accordance with state statutes and review work performed by unlicensed assistive personnel
• Coordinate continuity of care as indicated by patient and family needs in collaboration with the healthcare team
• Participate in process improvement activities to achieve clinical quality, customer satisfaction, and financial performance targets
• Act as a patient advocate and participate in committees and self-development programs
• Manage resources including personnel, supplies, and equipment to maintain a safe and efficient environment

What We're Looking For

Required:

• High School diploma or GED
• Active LPN license (Licensed Practical Nurse)
• BLS (Basic Life Support) certification from the American Heart Association
• Strong communication and critical thinking skills
• Comfort working with technology in a fast-paced environment
• Adaptability to change and enthusiasm for learning new processes

Preferred:

• 3-5 years of LPN experience
• Minimum 1 year of primary care experience

Schedule: Monday through Friday, 9:00 a.m. to 5:30 p.m., 40 hours per week. No weekends or holidays.

Location: United States (remote work may be available from specific states)
