On-site Home Visitor – Ft Stewart GA

Msccn is seeking an On-site Home Visitor to join our team in Fort Stewart, Liberty County, supporting military-affiliated families through in‑home visits and resource coordination.

About the Role

As an On-site Home Visitor, you will travel to residences within the Fort Stewart area to assess needs, provide information on available services, and help connect individuals with appropriate support programs. This role is ideal for candidates with a passion for serving veterans, transitioning service members, National Guard and Reserve members, military spouses, wounded warriors, and their caregivers.

What You'll Do

• Conduct scheduled home visits to evaluate the well-being and needs of military-affiliated individuals and families.
• Document observations, interactions, and service referrals accurately in case management systems.
• Provide guidance on benefits, counseling, employment assistance, housing, and other community resources.
• Collaborate with case managers, social workers, and partner organizations to ensure seamless service delivery.
• Maintain confidentiality and adhere to all relevant policies, including HIPAA and military family support guidelines.
• Prepare summary reports and follow up on outstanding actions to track client progress.
• Participate in ongoing training and professional development to stay current on best practices.

What We're Looking For

• High school diploma or equivalent; additional coursework in social services, psychology, or a related field is preferred.
• Experience conducting home visits, case management, or working with military families is a plus.
• Strong interpersonal skills with the ability to build trust quickly and communicate compassionately.
• Excellent organizational abilities and attention to detail for accurate documentation.
• Proficiency with basic computer applications, including email, word processing, and data entry tools.
• Valid driver’s license, reliable transportation, and willingness to travel within the Liberty County area.
• Ability to pass background checks and meet any required security clearances for working with military-affiliated populations.
• Commitment to upholding the values of respect, integrity, and service to the military community.
