About the Role

This position is part of PATH's Scattered Site Housing Team, where you'll work alongside case managers, housing specialists, and community partners to deliver comprehensive case management services. You'll use therapeutic and practical support strategies to help clients overcome barriers to stable housing and break cycles of homelessness. The role requires regular travel to clients' locations, making a valid driver's license and reliable transportation essential.

What You'll Do

  • Conduct thorough biopsychosocial assessments and develop individualized intervention strategies that address housing needs, mental health concerns, substance use, and other barriers to stability.
  • Build collaborative service plans with clients that set both immediate and long-term goals, regularly monitoring progress and adjusting support as needed.
  • Deliver ongoing therapeutic case management in field settings, providing consistent support to help clients obtain and maintain housing.
  • Work with supervising LCSWs to assess crisis situations and suicide risk, completing required incident documentation and coordinating appropriate interventions.
  • Navigate mainstream benefits applications, housing subsidies, and county assistance programs on behalf of clients.

What We're Looking For

Required:

  • Master's degree in social work or related behavioral health field (MSW, MFT, or equivalent) with eligibility for California Board of Behavioral Sciences licensure
  • Minimum one year of professional experience in healthcare, case management, homeless services, or direct work with vulnerable populations
  • Valid driver's license and reliable personal vehicle with proof of insurance; ability to qualify for PATH's vehicle insurance
  • CPR and First Aid certification (or willingness to obtain)

Preferred:

  • Current active licensure with the California Board of Behavioral Sciences (ACSW or LCSW)

You bring strong analytical and communication skills, the ability to collaborate across teams, ethical judgment under pressure, and a proactive approach to problem-solving in complex situations.
