What You'll Do
- Support clients with daily activities and personal care needs during overnight hours
- Partner with the care team to develop and implement individualized treatment plans
- Guide clients toward independent living and community-based programs through gradual transitions
- Provide counseling support and skills training to strengthen coping and resilience
- Document client observations and maintain accurate care records
- Monitor and respond to client needs throughout overnight shifts
