Key Responsibilities

• Deliver daily, community-based support tailored to each individual's goals and abilities
• Assist with personal care, household routines, and everyday living skills
• Encourage participation in social, recreational, and community activities
• Foster genuine, respectful relationships that promote dignity and independence
• Document progress and communicate openly with the care team and families
