About the Role
As an in-home health clinician with Signify Health in Durango, you'll practice at the forefront of home-based medicine. This part-time position allows you to see patients in their own environments, build meaningful clinical relationships, and make a direct impact on patient outcomes while working a schedule that fits your lifestyle.
What You'll Do
- Conduct thorough patient assessments and clinical evaluations in home settings
- Develop, implement, and adjust individualized care and treatment plans
- Order and interpret diagnostic tests; manage results appropriately
- Diagnose and treat acute and chronic health conditions
- Maintain accurate, timely clinical documentation and electronic health records
- Coordinate care with physicians, specialists, and interdisciplinary care teams
- Monitor patient progress and modify treatment approaches as clinically indicated
- Uphold quality and safety standards in all patient interactions
What We're Looking For
- Current, unrestricted Nurse Practitioner or Physician Assistant license in the state of Colorado
- Strong clinical knowledge, critical thinking, and independent decision-making ability
- Experience managing diverse patient populations and medical conditions
- Excellent written and verbal communication skills
- Commitment to patient-centered, compassionate care delivery
- Reliable transportation and willingness to travel throughout the Durango area for home visits
