Home Health Caregiver

Sevita is seeking a compassionate Full-Time Home Health Caregiver to join our team in White Bear Lake, Ramsey County. As a member of our mission-driven organization, you'll provide essential support that empowers individuals with intellectual and developmental disabilities to live independently and achieve their personal goals.

Working with individuals who have intellectual and developmental disabilities, you'll combine hands-on assistance with genuine human connection, building meaningful relationships that help your clients succeed both at home and in their communities. Day-to-Day Responsibilities

• Assist clients with daily living activities, including personal care, grooming, and hygiene
• Support meal preparation and nutritional needs
• Help clients with mobility, transportation, and community outings
• Promote independence by encouraging clients to develop skills and make choices
• Maintain a clean, safe home environment
• Communicate regularly with clients, families, and the care team
• Document observations and report any changes in client health or behavior
• Follow all care plans and safety protocols consistently
