About This Position
Health Coach Educators at Alignmenthealthcare empower individuals to take control of their health through one-on-one coaching, educational programs, and evidence-based wellness guidance. In this role, you'll work directly with members to identify health priorities, set achievable goals, and develop sustainable lifestyle changes that improve overall wellbeing.
Your Responsibilities
- Conduct individual and group health coaching sessions focused on nutrition, physical activity, chronic disease management, and preventive health
- Assess member health needs and create personalized health plans aligned with individual goals and clinical recommendations
- Provide evidence-based health education on wellness topics relevant to member populations
- Track member progress and adjust coaching strategies to support sustained behavior change
- Collaborate with clinical teams to ensure coordinated, member-centered care
- Maintain detailed documentation and report on coaching outcomes and member engagement
Who We're Looking For
- Health coaching certification or relevant health education credential (or willingness to obtain)
- Knowledge of health promotion, chronic disease prevention, and behavior change principles
- Strong communication and interpersonal skills to work effectively with diverse member populations
- Ability to educate members at various health literacy levels
- Passion for improving health outcomes and member wellbeing
- Experience in a healthcare or community health setting preferred
